Short Sleeve T-shirt Order Form

The Department is excited to be able to offer a UVA Ophthalmology short sleeve t-shirt for employees.  The short sleeve cotton crew t-shirts are unisex, will be ORANGE and have the UVA Ophthalmology “V” logo.

Price: $12.00 each (Cash or Check ONLY).

Please fill out form and click submit.  By submitting this order request, you are committing to the cost of the order. Cash or check payment is due at pick-up.  (Sorry, we cannot accept credit cards.)  If you want to order more than one, just fill out and submit a form for each order.  You will get a confirmation email for each order.  Please save or print for your own records.

Deadline to order is November 11, 2019.  Delivery will take 4-6 weeks.
