2025 Long and Short Sleeve Shirt Order Form

The Department is excited to be able to offer a UVA Ophthalmology Short Sleeve Shirt and Long Sleeve Shirt for employees.  Brand is Bella & Canvas Jersey Shirt.

The short sleeve cotton crew shirts are unisex, will be navy blue and have the UVA Ophthalmology “V” logo on the back and the UVA Rotunda logo on the upper front left side.

The long sleeve cotton crew shirts are unisex, will be navy blue and have the UVA Ophthalmology “V” logo on the upper front left side, and the word “Ophthalmology” running down the right sleeve.

Price:
Short Sleeve: $25.00 each

Long Sleeve: $30.00 each

All orders are Cash Only and Pick Up Only.

Please fill out form and click submit. By submitting this order request, you are committing to the cost of your order. Cash payment is due at pick-up. (Sorry, we cannot accept credit cards or checks)

If you want to order more than one, just fill out and submit a form for each order.  You will get a confirmation email for each order.  Please save or print for your own records.
