Using the Forms
These are electronic forms. When the form is completely filled out, click on the “SUBMIT FORM” button at the bottom of the page. The form will be submitted to the HIRE committee, and a confirmation email with a copy of the form will be sent to the contact email address provided.
Once you have submitted the form, wait to hear an acknowledgment from someone on the HIRE committee. Keep in mind- it may take up to 24 hours before you receive a response. If the HIRE committee needs more information or further clarification on the information submitted you will be contacted by one of us. At this point your trial/protocol has been submitted.
