Colonnades Clinic (CC)– 2600 Barracks Rd in Health Care Center
Start time: 8:30am or 1:00pm
Take N. Emmet St.
Take a LEFT onto Barracks Road
Go about one mile.
Take a LEFT into the Colonnades
At the stop sign, take your first RIGHT to the Health Care Center
JABA Clinic – 674 Hillsdale Drive (Clinics & Pre-OP)
Start time: 8:00am or 1:00pm
Take N. Emmet St.
Take a RIGHT onto Branchlands Blvd.
At the stop sign, Take a LEFT onto Hillsdale Dr.
Jefferson Area Board on Aging will be on your left.
Health South (HS) 515 Ray C. Hunt Drive in Fontaine Research Park
Start time: 8:30am
Take Jefferson Park Ave.
Make a LEFT to stay on JPA.
Turn LEFT into the Fontaine Research Park
Take a LEFT at the yield sign.
At the stop sign, take a RIGHT onto Ray C. Hunt Drive
Enter thru the 515 Building entrance.

Take exit 94 to US-340 towards Waynesboro/Stuarts Draft
Turn Right onto US-340 N/Rosser Ave.

Take N. Emmet St. (Rt. 29 N)
Take a LEFT onto US-33 W/ Spotswood Trl.
Take a RIGHT onto US-33 BR.
Take a LEFT onto William Mills Drive.
Facility will be on your Left.
Dogwood Village of Orange(DVOC)– 120 Dogwood Lane, Orange VA
Start time: 9:30am
Take 64E to the Gordonsville Exit (15N)
Turn LEFT onto highway 15N towards Gordonsville
Take a RIGHT at the traffic circle to stay on 15N
At the second traffic light after entering Orange, Take a RIGHT to stay on 15N
Go through the stop light at Main Street
After you pass the stop light at Spicer’s Mill Rd (Sheetz on Corner)
Take a LEFT onto Dogwood Lane.
Westminister Canterbury (WCBR)– 250 Pantops Mtn Rd
Start time 10:00am or 1:00pm
Take N. Emmet St.
Take a RIGHT onto 250 Bypass after Bodos Bagels.
Continue across the Rivanna River and up the hill.
Go past DMV (on your right) and get into the LEFT hand lane.
Turn LEFT at the second traffic light onto Pantops Mtn Road.
If you go past the Topeka Steak House you just missed the turn.
Go up the hill and through the security booth.
Take a left and try to find a parking space around back.
Go to the 2nd Floor of the Health Center

Take your next RIGHT into the parking lot.

Start Time: 9am- 3pm
Greene Co. Community Center (Greene)- 222 Main St. Standardsville VA
Take 29 N toward Culpepper for about 12 mile To US- 33W.
Continue to follow US 33 W. Turn tight onto 33 and go 4.7 miles.
At light, turn right onto US 33 Business Rt. Go about 1.5 miles, ceneter on left.
Louisa Co. Community Center (Louisa)– 522 Industrial Drive Louisa VA
Start time: 10am
Take I 64 E toward Richmond. Continue 18.4 miles.
Take UVA 208 exit toward Louise/Ferncliff
Turn Left onto VA 208, continue 8/9 miles
Turn Right onto US- 33/VA208/VA22/ E Main St. Continue 2.1 miles.
Turn Right onto Industrial Dr. Center is on the Right.
Charlottesville Health & Rehab– 505 Rio Road West, Cville VA 22901
Start time: 8:30 am
Take 29 N to Rio Road.
Turn Left onto Rio Rd. West
Building on your left.

East on W. Main Street to 12 1/2 St. NW
Keep left at the fork, continue on W. Main St.
Turn left onto Ridge McIntire Rd. (Becomes McIntire Rd.)
Turn right on US-250-Bypass East (0.1 miles past Allied Ln., if you’ve reached Hillcrest Rd. .1 miles too far)
Stay straight to go onto Long St.
Turn Right onto Peter Jefferson Parkway
If you reach Willis Dr. you’ve gone .4 miles too far.
