If you do not know the answers to the following questions please call the toll free number for benefits located on your insurance card and ask these three questions:

  1. Am I required to have a Primary Care Physician referral?  If the answer is yes then you must contact your Primary Car Physician (family doctor) and request that he call your insurance and make a referral for you to be seen by our physician.  You should keep track of these referrals as they are your responsibility and you will need to know when they run out.  If you need referrals and do not get them your insurance company may penalize you by not paying as much on your bill.  If the answer is no and you are not under managed care you do not need to do anything
  2. Ask your insurance company if the physician you are seeing in our department and the University of Virginia are in your network of providers.  Some insurance companies have contracts only with certain hospitals and will only pay if you go through their physicians and their hospitals.
  3. If you are having an outpatient procedure ask your insurance company if precertification is required for that particular outpatient procedure.  Some insurance  companies want to be notified before you have a CT scan, MRI, outpatient Lithotripsy, Brachytherapy, VCUG, Lasix Scan, Urodynamics or other procedure.  If you find out you may need precert for outpatient procedures please contact us and we will be happy to assist you in doing this.  We automatically precertify all inpatient admissions but do not precertify outpatient procedures unless the patient tells us this may be needed.

  • Any x-rays, medical records, pathology slides pertinent to your visit.
  • Your insurance card.
  • A list of your current medications or the bottles themselves.
  • If you are scheduled for Urodynamics and have mitral valve prolapse please check with your family doctor to see if you need to be premedicated.
  • Co-Payments (if necessary)
