Verify Medical Residency program

Below is information on how to obtain proof of certification and licensure from specialist departments.
Note: If you are attempting to verify a Medical Residency program completed at the UVa Health System, please contact the Graduate Medical Education Office.

The Office for Student Affairs can provide verification of your Diploma. To request this service, please complete the fields below. There is not charge for this service.
Certification of Medical School Enrollment (current students)

Replacement Diploma
Need a replacement for a damaged or lost diploma? The cost is $60.00 for domestic, and $70 for international, plus mailing fees. Contact the Office of the University Registrar.
MIDUS Participating Programs
We can upload your transcripts and MSPE through the MIDUS system. In order for us to do so, you must send an electronic request to the School of Medicine using EFDO On-line Services.
Steps for Submitting MIDUS Requests in EFDO On-line Services
- Log in to MyERAS and select “Additional Documents” from the Documents section.
- Click EFDO On-line Services, at the bottom of the “Additional Documents” page.
- Once your EFDO On-line Services page loads, click MIDUS Request at the top.
- Once you have carefully read the disclosures, check the box next to “I have read and accept the above disclosures.” and click Continue.
- Read the instructions and click New Document Request.
- Select which document type you are requesting (MSPE and/or medical school transcript). You must submit a separate request for each document type you would like uploaded through MIDUS.
- Enter complete contact information for your medical institution:

- Enter any comments or special instructions for your medical school.
- Carefully check the information you supplied, and click Submit. This will send the email request to your medical school.
- If your request was submitted successfully, you will see a confirmation page. Click OK on this page to return to the request homepage.
- Complete these steps for each document type you would like to request
You can track the status of your existing MIDUS requests from the request home page in EFDO On-line Services.
It is your responsibility to follow up with your medical school to ensure that documents are ready in time to be sent to EFDO to meet program deadlines.
MIDUS Non-participating Programs
For Transcripts
All official transcripts are ordered through the Office of the University Registrar. You can order online and have them sent by mail or electronically.
For your Medical Student Performance Evaluation (MSPE)
The Office for Student Affairs does not release copies of the MSPE to the former student directly, We are happy to mail or email the letter directly to the program. Please complete the form below to request this service.
Do you need a letter certifying that you are a current student? Do you need a licensure form filled out as a graduate of the School of Medicine? Do you need an Insurance Form filled out to verify that you are a “good student” to receive your discount?
Print your own Certificate of Enrollment
The National Student Clearinghouse (NSC) is the official provider of enrollment and degree certifications, and loan deferments for the University of Virginia.
We strongly encourage current students seeking certifications to take advantage of the speed and convenience of the National Student Clearinghouse online services.
Enrollment Certifications from NSC are considered official documentation of enrollment. They may be used for insurance companies, employers, scholarships, or any other agencies/services that require proof of enrollment at the University of Virginia. Receive your current and/or past enrollment or anticipated date of graduation information from NSC at any time. There is no charge for this service.

How to Sign Up
Students may signup online at:
https://www.nbme.org/examinees#close.
Our testing window is expected to be available in Mid-September.
After completing the online portion, the application will prompt you to download a copy of the “Certification of Identification and Authorization Form”.
Sign* that form and scan and submit below.
The Office for Student Affairs will print, including your official UVA ID photo and submit on your behalf to the NBME.
*Please do not use a font or digital signature.
USMLE Photo verification
Most states require that the School of Medicine complete educational verification paperwork. You can submit this paperwork through the tool below and we will complete and submit the forms. There is no charge for this service.
