These forms will be available on the SOMOGC website as well as OSP’s website (linked below). As always, activity already in process can proceed unchanged, but please utilize these forms for any new outgoing subaward transactions.

Dear Colleagues,
The Office of Sponsored Programs (OSP) in partnership with the Research Administration Improvement Team (RAIT) recently reviewed and updated the UVA forms used for outgoing subawards. The updated forms are available on our website here.
The Consortium Commitment Form is used at time of proposal, when UVA is the prime applicant and we will be entering into a collaborative Subrecipient relationship with an outside entity. The form has been updated to allow Federal Demonstration Partnership (FDP) Subrecipients to skip a majority of the form, reducing administrative burden. Please collect this form from all Subrecipients at the time of proposal.
The Subaward Request form, previously referred to as the SP-30, is the new, updated form that is sent to OSP to request a new subaward agreement, or to modify an existing subaward agreement. This form has been improved to include additional fields required for OSP’s review, facilitating faster turnaround time.
