MRI Screening Form for Research Personnel
For Research Personnel (Primary Investigators, Research Coordinators, etc.). It is vitally important that any research personnel who will be in the MRI Exam room complete this screening form to ensure safety while working in the MRI environment. The form can be emailed in advance to Jamie Weathersbee or brought in during the day of the exam.
