Clinical Facilities

The Department of Family Medicine’s main clinical and administrative offices are located on the first floor of the Primary Care Center (PCC), right in the heart of the University of Virginia Medical Center complex. Our clinic functions as an independent Family Medicine Practice, with a vibrant and caring International Family Medicine Clinic. We offer general and preventive care, including obstetrics and minor surgical procedures, to families and individuals of all ages.

Clinic Visits – In-person visits with a provider in Family Medicine are available by calling 434-924-5348.  We are ensuring your safety by taking appropriate precautions and screening measures.

Telemedicine Visits – You may be able to access care from home or another convenient location using your computer or smartphone.  Call us at 434-924-5348 to schedule a virtual visit with any member of our team including your primary care provider or one of our psychologists or psychiatrists. Please see additional information below.

Avoid a visit to the pharmacy. Get your prescriptions delivered to your home for free. To set up delivery, call 434-297-5500 to confirm your address and set up payment. Make sure to call at least 7 days before you need your refill.

Crossroads Family Medicine, situated at the crossroads of Route 29 and Route 692 in North Garden, offers general and preventive care, including obstetrics and minor surgery, to families and individuals of all ages. When hospital care is in order, we admit our patients to the Medical Center where they will be cared for by the Department of Family Medicine and by any specialists that may be needed.

UVA Health Employee Primary Care is a new clinic that will offer primary care services, including same-day appointments, for adult employees of the UVA Health University Medical Center and Academic division on the UVA Health Plan (all tiers).

Conveniently located on grounds, the clinic is an exclusive benefit for employees of any option of the UVA Health Plan. We aim to provide accessible, convenient, and first-rate primary care from UVA Health providers.

Stoney Creek Family Medicine, located on Route 151 in Nellysford, provides ongoing and comprehensive health care to families and patients of all ages. The practice was established by the Department of Family Medicine at the University of Virginia Health System to give the best possible care to the families in Nellyford and surrounding Nelson, Amherst and Albemarle counties.

UVA Family Medicine and Specialty Care Crozet, located 11 miles west of Charlottesville on Route 250 in “The Shoppes of Clover Lawn Shopping Center,” provides medical care to patients of all ages, including prenatal care and minor surgery. Should hospital care be needed, we admit to the University of Virginia Medical Center where our patients are cared for by the Department of Family Medicine.
