Welcome to the University of Virginia Anesthesiology Department’s Accredited Critical Care Fellowship in Critical Care Medicine (ACCM). Using a wide breadth of clinical experience in multiple specialized surgical critical care units as well as an immersive and tailored didactic program, our goal is to train future educators, innovators, and thought-leaders in critical care medicine.
Clinical experience within the one-year fellowship will include core rotations through the Surgical-Trauma Intensive Care Unit (STICU), Thoracic-Cardiovascular Surgical Intensive Care Unit (TCVICU) and Nerancy Neuro Intensive Care Unit (NNICU) ICUs at the University of Virginia Health System. We supplement the ICU-based experience with elective ICU rotations. These include the medical and pediatric ICUs, as well as non-ICU electives which may include cardiology/heart failure, pulmonology, radiology, infectious diseases, critical care nutrition, nephrology, palliative care, ECMO, research, and many others depending upon the individualized interests of our fellows.
UVA Anesthesiology’s Critical Care Medicine fellowship has a well-rounded and immersive didactic component that includes system-based pathophysiology, ultrasound, journal clubs, problem-based learning, board review, morbidity and mortality conferences, research, simulation, and hands-on courses. These include a critical care ultrasound program, ACLS/CALS, and ECMO simulations. Our fellows develop the insights and skills to manage ICU environments. They hone the skills to perform quality improvement projects and participate in the research which impact outcomes in the ICU.

The Critical Care Medicine fellowship at the University of Virginia provides an innovative, challenging, and in-depth critical care training opportunity in one of the country’s best medical centers. The fellowship is designed to meet the individual needs of each fellow in preparation for ACGME board certification in Anesthesiology Critical Care Medicine and evidence based critical care practice.
The University of Virginia Health System is a tertiary referral and Level I trauma center for central Virginia and parts of surrounding states. Currently, the critical care units at UVA include a 15 bed Surgical/Trauma/Burn Intensive Care Unit (STBICU), an 18 bed Thoracic/Cardiovascular Intensive Care Unit (TCVICU), and a 12 bed Neuro-Care ICU (NNICU). The TCVICU is staffed nearly exclusively by Anesthesia-trained intensivists, while the NNICU and STBICU has anesthesia, neurology, and surgical attending staffing that deepens the educational experience for the fellows.
The fellowship is an ACGME-accredited one-year program approved for Anesthesiology and Surgery trained physicians. We are an American Board of Anesthesiology APPROVED program for Emergency Medicine trained physicians as well, but EM fellows will complete a 24 month accredited fellowship program, which includes some surgery rotations, our core ICU rotations, and some emergency medicine rotations. The ACGME accredited Fellowship in Critical Care Medicine accepts up to four fellows annually through the San Francisco Match. The fellowship is under the direction of Danny Theodore, MD, Associate Professor of Anesthesiology. Core rotations make up 9 out of the 12 months in the critical care fellowship in accordance with ACGME regulations and are focused in Surgical/Trauma ICU, Thoracic/Cardiovascular ICU, and Neuro-ICU with an additional 3 elective months. The fellowship program offers a diverse array of electives such as MICU, PICU or a medical subspecialty (e.g., infectious disease, pulmonology, or cardiology) to help fellows tailor their educational experience to their future practice fields. Other popular rotations include ICU nutrition, TEE in the cardiac operating rooms, toxicology, ECMO, and the trach/PEG/nutrition service.
Fellows who have research interests are strongly encouraged to use their elective time to develop and pursue clinical or basic science investigations alongside their faculty mentors with opportunities to present their work at national conferences. Faculty at UVA are recognized leaders in the field with peer reviewed publications, book chapters, and speaking engagements at national conferences and will be valuable resources to any research pursuits or academic projects.
As a part of the daily responsibilities in the ICU, fellows are expected to organize and lead teaching/work rounds, formulate care plans with attending supervision, and facilitate communication between the ICU team and surgical services, consultative services, and family members. Fellows will also serve as instructors for intern and resident simulation/didactics, formulate problem based learning discussions from challenging ICU issues, and organize monthly journal clubs.
It is important that the education that the fellows gain from patient care is supplemented by a well-developed educational curriculum. This year-long teaching curriculum includes multiple weekly didactic sessions with both faculty from the division of critical care medicine, as well as experts within various specialties at UVA. In addition, fellows participate in simulation training at UVA’s Medical Simulation Center, a state-of-the-art multidisciplinary training and research facility.
Fellow simulation sessions are designed to be integrated with the monthly curriculum. The sessions are unique in that they do not only focus on a single event or crisis, but also simulate the progression of a disease process, such as sepsis or stroke.
The primary goal of the formal educational program is to provide a comprehensive, multi-disciplinary, evidence-based curriculum to our fellows. The program includes a weekly series of lectures that utilizes the content outline as stipulated by the American Board of Anesthesiology.
In 2020, our faculty helped begin a multi-institutional didactic program with Oregon Health University and the University of Wisconsin (The Ohio State University and University of Southern California have also participated.) These weekly lectures are given by many faculty speakers and guest physicians, who are leading experts in their fields. Unit leaders, NPs, PAs, and ICU care staff attend various lectures throughout the year with the fellows.
Weekly Round Table Discussions are hosted by UVA intensivists for the fellows to trouble shoot unit issues, discuss QI/research projects, career guidance, and short chalk talks.
We also have a monthly board review series (last Thursday on each month) to prepare fellows for the written examination. Fellows will also be required to complete the newly established American Board of Anesthesiology In-Training Exam for Critical Care Medicine.
